Aim a video camera at it.
Unless you use a second camera what you ask isn't possible. Unless you mean a simple screenshot still image: hold the power key down and then press the home key to take a single photo of the screen image.
Search for screen recorder on the App Store and try what comes up. There's a lot of trash on there so it might take a while to find a good one.
If you have a Mac, connect to computer & use QuickTime.
Otherwise there are apps (or were apps) but you'd have to pay for them
Sleep/Wake and Home button. Don't need a jailbreak.
If you're talking about recording gameplay or a span of time without a jailbreak, you'd need a tripod and a video camera.
