It's not supposed to, but it can happen. That is why you are warned to backup your phone before upgrading.

No, updating any Apple device to the latest firmware will not delete any data. It should be backed up to iCloud or a computer anyways. The only thing it would delete is your jailbreak if your phone happens to be jailbroken. This applies to iPod Touch and iPads as well.
