To heat up a little, yes that would be normal. But if it is heating up a lot then there's a serious problem. Especially if you have purchased a charger that is not genuine, for example cheap chargers from ebay imported from china. I have heard in the news just the other day that a lady was electrocuted whilst using her phone which was connected to the mains, and another person's socket burnt up.
So if you purchased a non genuine one and it is heating up a lot, then you should chuck it away and get a genuine apple charger. But if you already have a genuine charger that came with the phone and or you bought separately, then you still should replace it if it is heating up too much if it is faulty.
Well I have used my iPhone 4s on full charge and I can tell you it's gets very warm at 60% charge.
Yes it will get hot: if it's Ouch! Hot then it needs looking at or replacing
