Does the iphones 4s have a sim card slot?

And does this phone work only with a headset

Yes it has a SIM card slot no matter which phone company it's made for (AT&T or Verizon or Sprint or the factory unlocked version) - but on the Verizon and Sprint iPhone 4S, that SIM card slot is only activated when the phone is outside the USA.

It will work with a headset that's compatible with iPhone - but it also will work without one, just like any other phone.
