Does the ringer on an iPhone come from a different hole than what you hear a caller out of?

Yes, the "ringer" comes out of the main speaker/s at the bottom of the phone, you hear the caller out of the ear speaker unless you put it on loud speaker, then it'll be out of the main speaker/s.

The ear speaker isn't good enough to produce loud sound, if the ear speaker were to play your ringtone at the volume you normally hear it then it would most likely damage the speaker.

On the iPhone 7, 8, X, XS & XS Max (& the Plus models) they come from both the earpiece (what you hear a caller out of) & the speaker on the bottom right of the phone. (The one on the left is a microphone).

On the prior models only through the bottom speaker.
