Help my Iphone 5 I need to make an apple id without entering my creditcard?

There are only 4 choices Visa, Mastercard, Amex, and Discovery… I searched in itunes there's a word none but i can't find it in my Iphone 5?
Yo can make a apple Id online and then use it for your iPhone. But as far as I know there's one more option of billing which is by giving our address, if it's there choose that option and make the Id. Don't worry they won't send any bills, I have been using my apple Id since 2 years and all is perfect or instead for more easy why don't you add a credit card, they won't charge you anything without your approval.
- What happens after incorrectly entering a PUK code 10 times? My sister brought home an Iphone 4 or 4s from work a couple years ago after no one claimed it. I never touched it until now just looking through it. Well the Sim is locked ofcourse and there's no password on it. All I want to do is play games so what would happen if I put in the wrong PUK 10 times? Can it erase everything? Will it notify the carrier? Will it entirely lock me out? What happens if I factory set?
- No Service on iPhone 3 when entering a new sim card? I'm trying to set up this iPhone for my uncle. My mom gave it to him for Christmas, it was hers. It's in perfect condition and everything. I restored it, reset network settings 3 times. But I'm still getting the "No Service" thing. Is there any way to fix this without contacting his service provider? AT&T, by the way. Not a faulty sim card, either.
