HISTORY DELETES ITSELF ON IPHONE

So, my mom was messing around on my phone and now the history keeps deleting by itself! It's very annoying because I have to sign into my accounts again and again, and it never saves the password. Also, the two blue lines on the top and bottom of the screen turned black (i don't know what they're called, but even if that helps. I'm technologically impaired).So please help! Give me instructions?

I got my iPhone last year and had it synced to my laptop, however after a problem with the software everything was wiped from my laptop and new software was installed. I tried resyncing my phone and it says that it is already synced with another computer.
