SAM sim unlock iPhone 4S 6.1.3 T-Mobile

So I recently jailbroke my AT&T iPhone 4S running IOS 6.1.3
After jailbreaking it, I got a T-Mobile Micro SIM card activated to my number. Then I downloaded SAM from the Cydia app that is installed to the phone when jailbroken. I got the Phone to say the carrier selected is T-mobile (second picture) and to have the bars in the top left (first picture) but when I plug my phone into iTunes, it says the phone will not activate (third picture)
People on YouTube giving these tutorials did not have these problems. Please help!

Also, I waited to see if the T-Mobile symbol would pop up in the top left corner, yet it does not, and it does not let me text or make calls.

  • What is the different between iPhone T-Mobile Unlock and iPhone Sim-Free? The unlocked iPhone in US Store is T-Mobile contract free but in Canada or UK is unlocked and Sim-Free. So, What is the different?!

  • Reuse T-Mobile cut-to-use-for iPhone SIM card back to regular T-Mobile phone? Is that possible? My SIM card has been cut to fit on iPhone but I'm going to sell it next month to go back to regular T-Mobile phone. I still have the cutout piece from original T-Mobile SIM Card. Can I re-attach micro SIM to original T-Mobile card and use it on new T-Mobile phone, or would I have to buy the new SIM card and activate it again on pay-as-go-plan?
  • How much is the iPhone 5 at Sam's Club or Target? So I was in Sam's Club the other day and saw the iPhone 5 and I'm wondering how much it is as I did not see a price and did not have time to check. Also I saw an ad for one on Target's website for an iPhone 5 but again no price. So, does anyone know how much they are at Sam's Club or at Target?
