Unlock iphone 4 with AT&T online form?

Hi everyone i'm trying to unlock my iphone 4 with AT&T online form but i'm not on us anymore and i need some help. I need help with 2 options "Wireless Number" and Last 4 of SSN what do i put on those fields? Can somone just help me.
