How to backup All of my iPhone info onto my Macbook?

I thought I did this already but I don't think half of the stuff made it onto my Mac?!
I then want to transfer all of the info from my iPhone 4S to my Iphone 5. I thought I did this and yet NOTHING transferred over? What the heck am I doing wrong?
