What should my work out plan be?

I can't find an app that creates a workout plan for me weekly on my iPhone. Does anyone know of one? Or help me create my own

I'm looking to workout 4 days a week
I'm 19, 117 pounds, 5'0" and looking to go back down to my old weight 104 pounds in two months.

I'm mildly athletic, I played tennis and gymnastics in high school but now I'm a sophomore college and have low activity such as walking to class and doing crunches in my room. I enjoy the elliptical, treadmill, ab workouts, some weight machines. I know you can't lose weight in specific areas but I've gained a lot in my biceps, thighs, and lower stomach.

How much cardio should I do each workout? What should my workout routine be?
