The easiest way is to connect your iPhone to your computer, go into iTunes, click on your phone's icon at the top left of the screen, then click on "Apps". It'll show you each screen on your phone with its icons. Double click on the screen you want and move the icons around any way you want, even to other screens. When finished, click on "Apply" at the bottom right of your screen. Done.
