I don't think you just buy the SIM card. You have to buy service as well.
The SIM card must be issued by the carrier - in this case T-Mobile. You have to call them. If you already have service with them and are not changing phone numbers, a replacement should be free.

Didn't get you but if you're looking for a SIM for the iPhone 5, it requires a nano-SIM. You can take your normal or micro-SIM to T-Mobile and they'll trim the SIM card to nano-SIM.

If you try to buy a SIM at a T Mobile store, they will demand all of your account info and if you're not already signed up, they won't sell you a SIM unless you buy a plan right then and there. In other words, you can't just walk into a T Mobile store and buy a SIM to activate yourself online. They are not a customer-friendly company… It's their way or no way.
