If it is accidental such as dropping it or water damage I believe it does. However if it is a technical defect from the company it should be completely free, just need to make an appointment with the Genius Bar, they may try to fix it for you though rather than giving you a new one. I use to have a iPhone 3GS and got like 7 of them free through apple care.
If you have apple care, it only extends the manufacturer's warranty and tech support to 2 years.
If you bought apple care plus, then you also get one replacement of the unit for $50 if it suffers physical damage.
