Try a different set of head phones, if it is the phone, try restoring the software. Be sure to back it up first. I had this problem and it corrected it

Most likely it is just the ear phones, especially if they are they ones that come with the iPhone. They don't last very long. Try using another pair. If it still doesn't work, I suggest you backup your phone(Just in case) and take it to the Apple store, usually it's just that there's a bunch of lint inside the headphone jack.
It is usually the earphones that fail. Try another set.
