The SIM card does not hold all your info - it is not a memory card (though it does have the ability to store a very limited quantity of phone numbers - even then these would need to have been specifically saved to the SIM card!
Yes. However, nothing is stored on a SIM card except phone number and account info.
SIM cards don't store info, beyond your phone number & contacts (if you have them saved there (1 number per contact in that case)).
As far as SIMs between phones that's not an issue, except in the case of the other phone being locked to another carrier than the one the SIM is from.
