Press and hold icon app for three seconds until it starts shaking. Click the red X. And click delete.
It's a white X on a black circle background, on the top left of every icon corner when they're wobbling in edit mode. Approve the confirmation popup - "Deleting Facebook will also delete all of its data."
It's the same with iPhone, iPad, and iPod.
